Required Forms and Policy Documents

All Trips for Kids® chapters must complete the following forms.

  1. TFK Chapter Requirements: This is a minimum list of actions required by Trips for Kids on all rides. You may change the format of this form to match other forms that you use, but you may not alter the content (unless your policy is to have more stringent requirements). (For information on getting background checks for volunteers, click here.)
  2. Participant Statistics: All chapters must send in participant stats to Trips For Kids at the end of the year. Please use this sheet to record your statistics. To help you track statistics, you may want to use this trip log developed by TFK Marin and formatted by TFK Ponoco Flow.

Optional Forms

The information shared here comes from our chapters! To make this a valuable resource, feel free to send documents that you're willing to share with the Trips For Kids community and we will include them on this page.

  1. Waiver Form: a form that TFK Marin requires be signed by everyone who goes on a ride - parent or guardians of kids, as well as volunteers.
  2. Spanish Waiver Form: Marin waiver form translated into Spanish.
  3. Waiver Form - Charlotte: You may choose to use this waiver form. This form is more detailed and includes specific responsibilities for volunteers and personnel, background check information for volunteers and personnel, and more information from participants.
  4. What to Bring: This form is sent to the agency to distribute to the kids who will be riding.
  5. Agency Letter: Send this letter to agencies to confirm their ride date(s).
  6. Agency Invoice: This invoice is sent to the agency for payment.
  7. Volunteer Application: TFK Pocono Flow uses this form to gather information on potential volunteers.
